Summary
Vasopressin (argipressin) and desmopressin are related but distinct medicines, with different actions, indications, routes, units, and safety risks. Australian records include registered injection, nasal-spray, and sublingual products. Their dose schedules are not interchangeable, and no combined schedule is supported.
What the sources say
- In the TGA-reviewed placebo-controlled trial in 268 women, the change in mean nocturnal voids over 3 months was −1.46 with desmopressin 25 micrograms and −1.24 with placebo; the between-group difference was −0.22 (95% CI −0.42 to −0.02). The modest trial effect does not remove the need to consider hyponatraemia risk. (Human clinical trials; Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- In the randomized VASST septic-shock trial, low-dose vasopressin was added to conventional vasopressor therapy; 28-day mortality was 35.4% with vasopressin and 39.3% with norepinephrine, a non-significant difference. The trial did not establish a mortality benefit. (Human clinical trials; Russell et al., "Vasopressin versus norepinephrine infusion in patients with septic shock", 2008-02-28)
- The TGA registration for DESMOMED nasal spray lists treatment of ADH-sensitive cranial diabetes insipidus, selected primary nocturnal enuresis when alarm therapy is unsuitable or has failed and oral administration is not feasible, and diagnostic assessment of renal concentrating capacity. (Educator material (not peer reviewed); Therapeutic Goods Administration, DESMOMED (Medsurge Pharma Pty Ltd), prescription medicines registration record, 2025-08-22)
- Desmopressin is a synthetic vasopressin analogue with longer antidiuretic activity and little clinically relevant pressor effect at usual doses; vasopressin/argipressin has clinically important vasoconstrictor activity as well as antidiuretic action. They must not be treated as interchangeable. (Educator material (not peer reviewed); Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- The TGA assessment lists Nocdurna for adults with nocturia due to idiopathic nocturnal polyuria who awaken at least twice nightly and have not responded to lifestyle measures; the indication is product-specific and not a general antidiuretic or wellness claim. (Educator material (not peer reviewed); Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- The TGA desmopressin assessment identifies hyponatraemia as the principal serious harm, including potentially life-threatening water intoxication; fluid intake, sodium risk, comorbidities, and interacting medicines matter to safe use. The risks vary by formulation and indication. (Educator material (not peer reviewed); Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- Vasopressin/argipressin is a pressor medicine, whereas desmopressin is primarily antidiuretic at clinically relevant doses. Confusing their units or schedules could cause serious harm; the reviewed sources do not support substitution. (Educator material (not peer reviewed); Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- The Nocdurna dosing and indication details in this draft are drawn from the static TGA AusPAR dated 2017-11-27; check the current Product Information linked from the ARTG before review approval. (Educator material (not peer reviewed); Therapeutic Goods Administration, Australian Public Assessment Report for Desmopressin (Nocdurna), 2017-11-27)
- As checked on 2026-10-28, ARGIPRESSIN MEDSURGE argipressin injection vial was listed in the ARTG with active licence status; its ARTG entry date is 2025-08-19. This verifies that product entry, not an indication for every argipressin product. (Educator material (not peer reviewed); Therapeutic Goods Administration, ARGIPRESSIN MEDSURGE argipressin 20 pressor units/ injection vial (ARTG 449846), public ARTG record checked 2026-10-28, 2026-10-28)
- As checked on 2026-10-28, the TGA ARTG record for NOCDURNA 25 microgram desmopressin sublingual wafers showed active licence status; the ARTG entry date is 2016-12-02. (Educator material (not peer reviewed); Therapeutic Goods Administration, NOCDURNA desmopressin 25 micrograms (as acetate) sublingual wafers (ARTG 263596), public ARTG record checked 2026-10-28, 2026-10-28)
- As checked on 2026-10-28, the TGA ARTG listed DESMOMED desmopressin acetate nasal spray as an active registered medicine, ARTG 447745, with an ARTG entry date of 2025-08-27. (Educator material (not peer reviewed); Therapeutic Goods Administration, DESMOMED desmopressin acetate 10 microgram/actuation nasal spray (ARTG 447745), public ARTG record checked 2026-10-28, 2026-10-28)
- The TGA registration page records DESMOMED desmopressin acetate as registered on 2025-08-27 following a decision on 2025-08-22, with the listed nasal-spray indications described above. (Educator material (not peer reviewed); Therapeutic Goods Administration, DESMOMED (Medsurge Pharma Pty Ltd), prescription medicines registration record, 2025-08-22)
